PAC Required Paperwork 2026 - 2027
Please complete and sign each of the following required documents prior to the first day of care. Your child/children will not be allowed to attend PAC until we have received each of these documents.
The immunization record form will not be automatically submitted. If you fill out the form, please save and email it to pac@ftcpioneers.org
If your immunization records from a previous year are still accurate, please let us know and a new form will not be required. You may also email us your immunization record rather than filling out this exact form.
